What are your office policies concerning referrals?
Office referrals: We are a referral based practice. Please have your Primary Care Physician or current treating physician (Neurologist, Surgeon etc) call our office. Only after your doctor has faxed all pertinent records, lab and X-ray results (especially your most recent MRI) to our office will someone call to set up your appointment.
Insurance referrals: If you have an HMO insurance that requires a referral it is your responsibility to call your Primary Care Physician at least 3 days prior to your appointment. They will issue the referral and we will receive a faxed confirmation from your insurance carrier. You cannot be seen without a valid referral.
