What can I expect once I make an appointment?
While you are waiting for your first appointment, you are asked to complete a comprehensive History form so we can learn about your medical history and past pain treatments, if any. If time allows, these forms will be mailed to you prior to your first appointment so they can be completed at home and presented to us for review at your first appointment. These forms are very important, so they will have to be completed prior to seeing our practitioners.
You will have a complete Pain-related History and Physical evaluation by one of our practitioners and then Dr. Choquette will provide, in detail, an explanation of your working diagnosis and treatment options. Depending on what the first treatment step is, a follow-up appointment will be made so we can re-evaluate your condition and determine the next step in your treatment plan depending on your response to the first step. Every patient is different so we try not to provide “percentages” of patient’s that respond to a particular treatment. We treat every patient individually and determine what is right for YOU depending on the symptoms that YOU present with and how YOU respond to treatment.
