We are a referral-based practice. Please have your referral doctor or primary care physician contact us.

The office has compiled the following frequently asked questions and answers regarding appointments and payment policies. Please read these carefully. Not knowing the answers in advance can cause a delay in your treatment.
- Will I be charged if I miss an appointment?
Yes. There will be a $75.00 charge for missed office appointments and $125.00 for a missed procedure. We require 24 hours notice to cancel or reschedule your appointment. - Will someone call to remind me of my appointment?
No. Please do not depend on us for a REMINDER. Staff availability and time permitting, we try to call and confirm appointments a day in advance. This is done strictly as a courtesy. Not receiving a “reminder” call is no excuse for missing your appointment. - What happens if I’m late for my appointment? Will I still be seen?
Several things can determine the answer to this question. If you arrive more than 15 minutes late you forfeit your appointment time. Depending on the type of appointment, the time allotted and whether or not our schedule allows, you will either be given the opportunity to wait or asked to reschedule your appointment. If you wait you will be seen at the first available opening as our schedule permits. So please try to be on time for your appointment. - Will you call my doctor’s office when I need a new referral?
No. I t is your responsibility to get a referral from your PCP (primary care physician). If you arrive for your appointment without your referral you can call your PCP’s office and try to have the referral FAXED to us or reschedule your appointment. You will not be seen without a valid referral. - Can you bill me for my co-pay after my insurance pays?
Yes but a $5.00 charge will be applied to your account to cover the cost of billing if you do not pay at the time services are rendered. You will be expected to pay your co-pay at each appointment - Can I pay by check or credit card when I come in?
Yes . We accept Master Card, Discover Card and Visa ( credit & debit) and personal checks for payment on your account. There is a $25.00 penalty charged for any returned check. - Can I continue treatment if I owe a balance on my account?
Payment is expected at time of service unless prior financial arrangements are made. As we receive payment from your insurance, a statement will be sent to you showing the portion you owe . The payment of this unpaid balance will be expected within 30 days from the billed date. You can call the billing office (610-439-2844) prior to your appointment if you need to set up a monthly payment plan. Interest charges will be added to the unpaid balance of your account at a rate of 1.8% per month. - Can you tell me if my insurance covers Pain Management?
It is your responsibility to verify your insurance benefits. You need to call member services at the number on the back of your insurance card and verify your benefits for Pain Management. Please feel free to call the billing office at 610-439-2844 prior to your initial appointment and someone will try to answer any questions you may have regarding your benefits.
